Patient Rights & Responsibilities

As a patient and our guest, you have the following rights and responsibilities:
Patient Rights:
- To be treated with respect and dignity at all times
- To have healthcare providers communicate tailored to your understanding
- To have benefits, risks, possible outcomes and alternatives explained for any diagnostic and/or therapeutic procedure performed or considered in your plan of care
- To participate (with a support person’s assistance) in the development and implementation of your plan of care and be fully informed when it is necessary to be discharged or transferred from one accommodation to another facility
- To receive care in a safe setting and to be free from all forms of abuse and harassment
- To receive confidential treatment of all communications and records concerning your care (except when laws require otherwise) and to access information contained in your clinical records within a reasonable time frame
To receive reasonable attention to complaints, grievances and ethical issues when communicated to the staff without fear of reprisal, discrimination or deprivation of any kind
- To examine and receive an explanation of your bill regardless of the source of payment
- To refuse treatment, medication and participation in any research to the extent permitted by law and to be informed of the consequences of such action
- To have appropriate and regular assessments of pain and to develop a plan of care to manage your pain
- To maintain any social, religious and community interrelationships at your discretion
- To execute an advance directive relating to your requests concerning life-sustaining measures
- To have a family member or support person and physicians of your choice notified of your admission to the hospital and of significant changes to your condition
- To be fully informed of your medical condition and outcomes of medical treatment, including unanticipated outcomes
- To have a family member, friend or other individual present for emotional support during your stay and to refuse visitors (MMH may impose visitor restrictions or limitations when clinically justified.)
Patient Responsibilities:
- To be fully informed of any diagnostic and/or therapeutic procedure before signing consent forms
- To follow the directions and orders of the physician
- To voice concerns about nursing care directly to the nursing manager on the unit
- To report any and all perceived risks in care to the physician, staff or nursing manager on the unit
- To follow the rules and regulations of the hospital
- To follow the advice of the physician as to appropriate discharge time
- To discuss with the physician any questions regarding any aspect of treatment
- To discuss your pain management plan with your doctors and/or nurses and to keep them informed of effective pain relief measures, as well as continued pain that is unresponsive to pain management techniques

Written grievances should be mailed to 321 Mitchell Ave., Batesville, IN 47006.
