MMH may participate in some, but not all, products offered by a health plan. Coverage limitations are dependent on individual group contracts. While we strive to maintain consistency in our network participants, please be aware insurance coverage may be different at MMH physician practices or other physician offices.

If your insurance company is not listed, it does not mean you cannot utilize services at MMH. However, we suggest you explore any out-of-network penalties your insurance carrier may impose. You will be responsible for deductibles and/or co-payments that apply. We request these payments be made during your pre-registration or at the time of service.
Fees charged by providers such as radiologists, anesthesiologists or pathologists are not included within the MMH bill. You will be billed separately for their services.
