This information is especially crucial in case of emergencies or when seeing a new doctor. It ensures you receive the appropriate care and avoid any potential drug interactions. Read on for tips on how to effectively manage and use your medication list.

  • List every prescription medication, over-the-counter medication, vitamin, supplement, herbal product and traditional cultural medicine you are currently taking.
  • List medications you have stopped taking because of allergic reactions or for other reasons, like side effects, effectiveness or cost.
  • Include medication details, including the reason it’s taken, dosage, directions and prescribing provider.
